Identifying & Reporting Safety Events
Text Lesson
The Importance of Categorization
In this module, we define patient safety events and explore the different categories of safety
incidents that can occur in a healthcare setting. Understanding these definitions is crucial for
accurate reporting and prevention. To make these concepts clear, we will follow the case scenario
of Mrs. Adjoa Obeng, a patient at high risk for falls, throughout her hospital stay.
Study Duration
5 Min
Not all safety incidents are the same. We categorize events into five distinct types to ensure they
are handled with the appropriate level of urgency and analysis. Categorizing these events is vital
because it:
• Enables Proactive Prevention: Identifying hazards allows us to fix problems before they
reach a patient.
• Highlights "Safety Nets": Tracking near misses shows us where our double-check
systems are working well.
• Prioritize Resources: Sentinel events trigger urgent investigations due to their severity.
• Supports Data-Driven Growth: Grouping data helps the organization identify trends for
continuous quality improvement.
